"My child(ren) has/have my permission to participate in the St. Gerard Majella Parish Sports program. I understand that St. Gerard Majella Parish, including the St. Gerard Majella Club (SGM Sports), any of its coaches, officers or members, carries no medical, accident or liability insurance of any type. I acknowledge and agree to these terms and that it is possible my child(ren) may sustain an injury while participating in these activities and, in the event that such an injury occurs, St. Gerard Majella Parish, the St. Gerard Majella Club (SGM Sports), its coaches and any of its officers or members shall have no liability or obligation as a result and I hereby specifically release and discharge them and agree to hold them harmless from such liability. Please notify the appropriate coaches of any potential or relevant medical (including mental/physical/health) conditions or ailments, pertaining to your child(ren). Lastly, I agree to abide by CYC rules and regulations."
