ACKNOWLEDGEMENT OF RISKS & ACCEPTANCE OF RESPONSIBILITY RELEASE OF LIABILITY


BY CLICKING AGREE BELOW I ACKNOWLEDGE AND AGREE TO THE FOLLOWING:

In consideration of being allowed to participate in any way in the FAAA Sports Programs, its related events and activities. I the undersigned (BELOW), acknowledge, appreciate, and agree that:

  1. I. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular skills, equipment and personal discipline may reduce the risks, the risk of serious injury does exist; and,
  2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, assume full responsibility for my participation: and,
  3. I willingly agree to comply with stated and customary terms and conditions for participation. it however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the FAAA immediately; and,
  4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS, the FAAA, their officers, officials, agents and/or employees, other participants, sponsoring agencies. Sponsor advertisers, and if applicable, owners and lesser of premises used for the activities ("releases"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to personal property associated with my presence or participation, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

BY REGISTERING HERE, I ATTEST I HAVE READ THE ABOVE RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS AND VOLUNTARILY ACKNOWLEDGE THIS WAIVER AND RELEASE OF LIABILITY WITH OUT ANY INDUCEMENT BY COMPLETING THIS ONLINE REGISTRATION.

FOR PARENTS/GUARDIANS OF PARTICIPANTS OF MINORITY AGE (UNDER 18 YEARS)
This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releases', and for myself, my child and our heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releases from any and all liabilities incident to my minor child's involvement or participation in these programs as provided above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law.


PHOTOGRAPHY: In addition I understand that while participating in this kind of activity or related activities that the participant may be photographed. I understand and agree that the photograph(s) may be used in whole or in part, at any time, in composite or distorted form, in color or otherwise and may be displayed through any medium including, but not limited to printed medium, video, or on the internet. By signing this waiver I authorize any initial and any subsequent disclosure or publication of the photographs) at any time.

PARENT CODE OF CONDUCT


BY CLICKING AGREE BELOW I ACKNOWLEDGE AND AGREE TO THE FOLLOWING:

  • I will inform my my coach or team manager fi my child is going to miss or be late to practice or games
  • i will remember that playing baseball/softball should be fun and enjoyable
  • I will do my part to help maintain a positive baseball/softball experience
  • I will remember baseball/softball is a team sport; We win as a team and lose as a team; There is no I in team; No gamee is won or lost by one player.
  • I will develop my child's appreciation for the game
  • I will not coach my child from the sideline — the players need the opportunity to play their own game and the coach needs to be the one to instruct the players at the appropriate times.
  • I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and officials at every game and practice. (I will respect and not argue with the umpires, players, or coaches.)
  • I will not criticize or teach my child to criticize others when mistakes are made. We all make mistakes. We are human. NO ONE IS PERFECT.
  • I will be respectful towards other parents and players. I will remember this is a youth baseball/softball game. I will know and follow the rules and policies set forth by my Team, FAAA, Missouri. River Baseball League, and the North County Travel League.
  • I accept and will follow the parental code of conduct and help provide my child, our team, and FAAA with a positive baseball/softball experience.

I understand any violation of the Parent Code of Conduct may be subject to board review and could lead to board action leading up to possible loss of park privilege's.

PARK POLICIES AND RULES


BY CLICKING AGREE BELOW I ACKNOWLEDGE AND AGREE TO THE FOLLOWING:

REFUND POLICY

The Executive Committee has the sole authority to grant refunds only after review at its next regularly scheduled meeting.

A copy of the original registration receipt is required.

    Refunds are granted only under specific guidelines:
  1. Participant is physically unable to participate due to injury. (DOCTORS NOTE REQUIRED)
  2. Participant's family is relocated.(PROOF IS REQUIRED)
  3. The Association is unable to provide a team or league for the registrant to participate.
    Refunds are not granted for the following reason:
  1. Participant chooses to change team, divisions, or leagues.
  2. Parents disapprove of Manager, Coach, team members, or parents of players.
  3. No copy of registration receipt.
  4. After the completion of the first (1st) regularly scheduled league game.
  5. Participant(s) or parents of participant(s) disagree with established policies, practices, or guidelines set forth by the Association.

TO START THE PROCESS OF A REFUND CONTACT THE APPROPRIATE COMMISSIONER OF THE SPORT BEING PLAYED.


FOOD & DRINK POLICY

At the Florissant Area Athletic Association our concession stand is what runs our park.

Our policy is NO OUTSIDE FOOD OR DRINKS ARE ALLOWED to be brought into the ball park.

We do ALLOW players to bring in WATER or GATORADE for them to drink.

Managers are responsible for any outside food or drinks that is brought in the ball park from parents on the team.

If outside food or drink is found the manager of the team will get one warning to resolve the issue. if the issue is not resolved the manager will forfeit the game.

We are sorry for doing this but as we said the concession is what keeps our park running and our prices reasonable. We thank you for time in is matter. And please remember this park is run by volunteers.

PET POLICY

At Florissant Area Athletic Association, we DO NOT ALLOW PETS. We know you love your pets and your dog may be friendly. However, for the safety of all visitors and participants at the ball park, please do not bring your pets to the park.

By clicking AGREE below I ACKNOWLEDGE and hereby AUTHORIZE FLORISSANT AREA ATHLETIC ASSOCIATION, INC. to charge the credit card indicated in the following Payment web form for player registration and any other sports related fees, for the amount indicated in the Shopping Cart and is valid for one time use only. I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company; so long as the transaction corresponds to the terms agreed to this electronic registration. The charges will appear on my credit card billing statement as Youth Sports Registration.

Convenience Charge:
I further UNDERSTAND and AGREE that a Player Convenience Fee may be added to my shopping cart at checkout. This Fee(s) is required in consideration of the service fees incurred for online player registration and/or acceptance of credit cards as a form of payment. If you are charged a Player Convenience Fee, the Fee will appear on your receipt as "Taxes" (which is the subtotal of Convenience Fees in addition to any Taxes that may required by law.) The Service is intended to be used by adults who can make purchase with a credit card. If you are under 18, you may use our online registration program only with involvement of a parent or guardian.

Termination:
You may terminate this Agreement at any time by providing written notice to us.
