You must be registered with Circle of Concern
You must first Register on our website and choose form of payment CHECK
You will receive an e-mail confirmation showing the amount due.
Print your e-mail and submit to Circle of Concern at your next appointment.
All applications for the Circle of Concern must be presented to them no later than March 1, 2012
Acceptance and availability of funds are not guaranteed and may be limited
