Required Paperwork to complete registration - Thursday, December 17, 2015

The following paperwork will be needed prior to the first tryout for your registration to be complete:

Birth certificate

Medical release form

Player Verification form

Volunteer form with picture ID- Parents

Parent Code of Conduct

3 proofs of address

You will need to choose 3 of the following 17 proofs dated FEBRUARY 1, 2015- JANUARY 31, 2016:

 1. Driver’s License

2. Voter’s Registration

3. School records

4. Welfare/child care records

5. Federal records

6. State records

7. Local (municipal) records

8. Support payment records

9. Homeowner or tenant records

10. Utility bills (i.e., gas, electric, water/sewer, phone, mobile phone, heating, waste disposal)

11. Financial (loan, credit, investments, etc.) records

12. Insurance documents

13. Medical records

14. Military records

15. Internet, cable or satellite records

16. Vehicle records

17. Employment records

NOTE: Example – Three utility bills (three items from No. 10 above) constitute only as ONE PROOF

T-Ball  League

T-Ball Division is for boys and girls 4–6 years old

Minor League
The Minor League Division is for boys and girls ages 7–12.

Major League
The Little League Division is for boys and girls ages 11–12.

Junior League
The Junior League Division is a program for boys and girls ages 13–14.

Senior League
The Senior League Division is for boys and girls ages 14–16,
